Authorization form

All health professionals must be respectful of confidential information while functioning within the demands of close interdisciplinary collaborations. Therefore, it is not uncommon for us to have to validate a detail with your treating physician or to obtain some test results that you may have had.

Also, if you are under the care of a third party provider (CNESST, SAAQ,…), the law dictates that we provide certain reports.
